Overview

Incidents can result in or have the potential to result in injury or illness, damage to property or vehicles or to third parties. When an incident happens what is your agency’s reaction? Conducting incident investigations can identify the causes of the incident and will help to prevent them from reoccurring. Effective investigation focuses on the root cause and better enables targeted implementation of risk controls.

“Carrying out investigations will provide you with a deeper understanding of the risks associated with your work activities. An effective investigation requires a methodical, structured approach to information gathering, collation and analysis. The findings of the investigation form the basis of an action plan to prevent the incident from happening again and for improving your overall management of risk.”
